- hip fusions acn occur spontaneously following childhood sepsis or after ORIF of acetabular fractures (secondary to heterotopic bone).
- they also occur spontaneously due to ankylosing spondylitis;
- surgical fusions are performed for young adults with advanced arthritis;

- desire to return to near-normal physical activity with manual labor;
- 20 yrs years after surgery, 80% of pts w/ hip arthrodesis performed at relatively young age were working & satisfied w/ their results;

- Discussion:
- arthrodesis provides potential of a painless, stable base of support but it does so at the expense of knee motion;
- optimal position of arthrodesis:
- slight valgus, 10 deg of external rotation, and 0-20 deg of flexion (knees that have been shortened due to previous arthroplasty should be fused in full extension);
- valgus alignment with slight flexion easier to obtain with an external fixator than w/ IM nail;
- IM nails tend to cause 2-5 deg of varus;
- note that if future total knee replacement is a consideration (fusion takedown and arthroplasty) then it is important that the patella not be included in the fusion;

Knee Fusion for Irretrievably Failed Total Knee Replacement
Over the past 30 years, the increasing success of total knee arthroplasty has gradually replaced arthrodesis as the procedure of choice in most conditions causing severe joint destruction. Instead of a primary procedure, arthrodesis is now considered a salvage procedure relegated to cases where total knee replacement may he contraindicated, e.g. sepsis, neuropathic joint, and youth, or for an irretrievably failed total knee replacement

Triple Arthrodesis eMedicine Orthopedics
A triple arthrodesis consists of the surgical fusion of the talocalcaneal (TC), talonavicular (TN), and calcaneocuboid (CC) joints in the foot. The primary goals of a triple arthrodesis are to relieve pain from arthritic, deformed, or unstable joints. Other important goals are the correction of deformity and creation of a stable, balanced plantigrade foot.

Wrist Arthrodesis eMedicine Orthopedics
A painful wrist causes disability and decreased function of the upper extremity. The articulation afforded the hand by the wrist is important for mobility, strength, and dexterity. The pain produced by arthropathy of the wrist causes the patient to use a wrist splint to decrease pain. However, loss of mobility due to splinting is only part of the problem. The pull of the finger and wrist flexors and extensors exerts considerable force on the many intercarpal joints. Pain produced by bone-on-bone contact is compounded by the pressure exerted in active motion, and the result is reluctance of the patient to perform active motion. The goal of wrist arthrodesis is to provide the patient with a relatively pain-free wrist by eliminating movement in arthritic joints.
